Use it in Google Sheets or Excel

The download is a plain CSV file, so it opens in anything that reads spreadsheets.

Google Sheets: open a new sheet, choose File, Import, Upload, and pick the downloaded file. Choose "Replace spreadsheet". Then File, Make a copy if you want a clean original.
Excel: double-click the file, or use File, Open. When you save, choose "Excel Workbook (.xlsx)" so your formatting and any formulas you add are kept.
Numbers (Mac): File, Open, then pick the CSV.

The first data row is an example so you can see the format. Delete it before you start.

What to write in a car maintenance log

A useful log answers two questions later: what was done and when is the next one due. For every entry, write down:

Date and odometer reading. Most service intervals are "every X miles or Y months, whichever comes first", so you need both.
What was done. Be specific: "front pads and rotors" beats "brakes".
Parts and fluids. Brand, part number, oil weight, quantity. Next time you'll know exactly what to buy.
Cost. Parts and labor. It adds up to the real cost of owning the car.
Who did it. You, or which shop. Keep the receipt with the log if it came from a shop.
Next due. The mileage and date for the next service of the same kind.

What should a car maintenance log include?

At minimum: the date, the odometer reading, what was done, the parts and fluids used, the cost, who did the work, and when the next service is due. The template has a column for each, plus a header for the vehicle, VIN, oil type and tire size.
